The trade-off is specialization, because it will only feel useful to people working in that workflow and it does not try to be a full medical records system. In practice that usually means forms, repeated entries, and the discipline of entering things the same way every time. Compared with a plain memo app, it is much more purpose-built; compared with a hospital EMR, it sits as a focused side tool. The medical category fits it well, and that narrowness is what gives it value.
